Tensions exploded in the fifth inning of Tuesday's game between the Kansas City Royals and the Baltimore Orioles after Royals starter Yordano Ventura hit Orioles shortstop Manny Machado with a pitch.

Per Roch Kubatko of MASN Sports, Machado charged the mound and threw a punch after Ventura plunked him with a 99 mph fastball. The two had exchanged words earlier in the game.

San Francisco Giants star catcher Buster Posey was removed from the lineup ahead of Tuesday's game against the Boston Red Sox in order to deal with an inflamed nerve in his right thumb, per Andrew Baggarly of the San Jose Mercury News.

Los Angeles Angels pitcher Tim Lincecum is bound to make his debut with the club soon enough, but he said Tuesday he'll have one more outing for the Triple-A Salt Lake Bees before returning to the MLB level.

Major League Baseball unveiled the second update for the leading vote-getters for this summer's All-Star Game, with the Kansas City Royals and Boston Red Sox currently dominating the voting.

Kansas City catcher Salvador Perez is currently the top vote-getter. MLB Communications shared the updated results:  

Colorado Rockies shortstop Trevor Story and Los Angeles Dodgers shortstop Corey Seager have both broken the MLB record for most home runs before the All-Star break by a rookie shortstop, per Elias Sports Bureau (via ESPN Stats & Info).

The St. Louis Cardinals announced Monday that they have optioned second baseman Kolten Wong to the Triple-A Memphis Redbirds. 

Instead of promoting someone else from the minors to fill Wong's spot on the roster, the Cardinals expect veteran infielder Jhonny Peralta to come off the disabled list Tuesday.

Miami Marlins president of baseball operations Michael Hill announced Sunday that 2015 first-round draft pick Josh Naylor injured minor league teammate Stone Garrett with a knife during an incident at their apartment, per Craig Davis of the Sun Sentinel.

According to Davis, Garrett needed three stitches to close a cut on his right hand.
